**The service you will work in.**

Based in the Oremi centre, we enable service users to avoid and in particular to reduce the need for, prolonged hospital admissions and long-term dependency on mental health services. We promote recovery and prevent social exclusion among service users with mental health support needs. This service delivers targeted community based, person-centred and holistic support to service users who are of Arabic speaking heritage, aged over 18, who have a severe and/or enduring mental health needs and living in the Borough of Westminster.

Our practice is guided by the principles of the Recovery Approach. It is our aim to help people recover from mental ill health, and we work hard to help people return to as full an involvement in everyday life as they are able. We particularly help people to redevelop their strengths in returning to employment, education, and a full social life.

**A day in this role looks like.**

As a Outreach Support Worker at Hestia, you will play a crucial role in our Complex Needs service Your contributions will directly influence the lives of our service users, guiding them towards a path of empowerment and independence.

A day will never look the same and some of the things you can expect to do/be responsible for are:

- Be responsible for the support and well-being of a group of key service users allocated from across the team using holistic, person-centred support planning with SMART targets and using the recovery approach to enable them to increase independence, maintain their accommodation and enjoy a good quality of life.
- Visit service users for support sessions within their homes or appropriate community locations and enable them to access medical services and the local community mainstream services.
- Minimise unplanned and emergency hospital admissions for mental health relapse. To formulate, implement and review support plans with service users and associated professional participation.
- Liaise effectively with other agencies involved in the support of service users where appropriate.
- Provide information and advice on benefits, housing and welfare issues to service users.
- Assist the Outreach Team Leader to develop strong working partnerships with local providers particularly within universal and mental health service settings.
- Support users in accessing mental health resources, enabling access to opportunities for recovery and rehabilitation and community capacity building.
- Work with the Outreach Team Leader to review the social inclusion progress of the service users addressing equality and diversity issues for Black and minority ethnic communities or for people who identify as lesbian, gay or transgender, ensuring confidentiality is kept at all times.

- Fluency speaking in both Arabic and English.
- Experience of working with people requiring support related to mental ill health and complex needs.
- Experience of delivering support to people within a community/outreach setting.
- Ability to support service users to achieve their goals & aspirations.
- Excellent knowledge of issues related to signs, symptoms and treatments of mental illness; ability to assess service users declining mental health and take appropriate action.
- Knowledge of health and safety relevant to the role including working within a community setting.
- Knowledge of key working, person centred support planning systems and the recovery approach.
- An understanding of equality and diversity and how it applies to the job role and the importance of promoting service users' rights, privacy dignity and choice.
- Knowledge of welfare benefits entitlements appropriate to the user group.
