- Job description**Post Ref**: CC612- We have an excellent opportunity for a creative and motivated individual to join the Post Discharge Assessment Team, as a service co-ordinator. The team sits within the Prevention and Early Help Service, which is part of the Adult Services and Wellbeing Directorate at Calderdale Council. Our role is to work with and support adults from the age of eighteen with Care Act assessments where needed. Focus is given to those who have recently been discharged from hospital to either their own home with home care support or to a short-stay care home placement whilst awaiting a further period of assessment.- Calderdale Adult Services and Wellbeing believe in ensuring the health and well-being of adults living within the borough. Many have increasingly complex needs or frailty and are often more vulnerable members of society. We recognise how the interaction between physical, mental, and social care factors is important to ensure the independence of adults wherever possible and their quality of life is not compromised.- Calderdale’s approach to social care is to be creative, proactive, preventative, and personalised with a focus on holistic strengths-based assessments. The council is passionate about engaging with adults, carers, and their families to ensure a personalised approach is at the heart of every conversation.- As a service-coordinator you will have a pivotal role as part of the social work team in supporting individuals to maximise their potential and improve their quality of life and overall wellbeing.**The role requires you to be**:

- An effective communicator and active listener as you will support individuals, carers, and families which requires understanding their lived experience.
- Excellent organisation skills as you will be required to manage your own time and casework to ensure tasks are prioritised daily.
- Confident in working with other stakeholders as part of a multi-disciplinary team.
- Good record keeping skills as you will be required to make accurate case recordings, type factual assessments and detailed support plans.
- Ability to assimilate information and make professional judgements as you will be required to support safeguarding enquiries and raise any complex issues particularly those involving risk to your management team.
- You will need to be a caring and compassionate person with a willingness to help and support others and demonstrate emotional empathy to be successful in this role.
