The post holder will be responsible for the assessment, examination, diagnosis and treatment of cardiothoracic patients to an advanced level. This must include engagement, consultation and negotiation with patients at the centre. At the development level, the post holder will be working towards this whilst undertaking the necessary education requirements.

The post holder is required to deliver/work towards an advanced level of practice within the field of cardiothoracic surgery and the scope of practice as defined by the individual post’s role outline. This is provided separately.

This definition therefore requires that health and care professionals working at the level of advanced clinical practice will exercise autonomy and decision making in a context of complexity, uncertainty and varying levels of risk, holding accountability for their decisions.

The post holder will have/working towards a Level 7 award at Post Graduate Diploma (PgDip) or full Masters (MSc) in Advanced Clinical Practice, with an approved Higher Education Institute (HEI). The Trust may recognise and accredit prior learning and practice depending on the currency and quality of the evidence within the four pillars of Advanced Clinical Practice in support of equivalence.

4. Duties and Responsibilities 4.1 Clinical Practice

1. Practice in accordance with the respective code of professional conduct and within designated scope of practice, responsible and accountable for own decisions, actions and omissions at this level of practice.

1. Demonstrate a critical understanding and adhere to the defined boundaries of advanced practice in this role.

1. To undertake/work towards advanced clinical practice as defined by the Health Education England (HEE) Framework and in doing so act as a source of expertise to others, in order to provide specialist multi-needs assessments using complex and advanced clinical reasoning demonstrating critical thinking, reflection and analysis to develop individual and group goal and task-oriented treatment plans.

1. Be a source of expert opinion for other staff across professional boundaries.

1. To work autonomously and manage a caseload of patients within the cardiacthoracic wards whilst working as part of the multidisciplinary team, delivering individualised and personalised patient care.

1. Have interpersonal skills that encourage patients and carers active participation in their care. Have advanced verbal and non-verbal communication skills. Be able to receive and disseminate complex clinical information sensitively in all professional contexts, adapting to cultural and socioeconomic complexity.

1. The practitioner will use and demonstrate/work towards advanced sensory skills in the examination and assessment of patients with complex health conditions with narrow margins for error and demonstrates highly developed physical skills through clinical procedures and treatment where accuracy is important.

1. Use expertise and decision-making skills to inform clinical reasoning approaches when dealing with differentiated and undifferentiated individual presentations and complex situations, synthesising information from multiple sources to make appropriate evidence informed judgements and diagnoses.

1. To initiate, evaluate and modify a range of interventions which may include prescribing medicines advanced treatments and investigations.
