**CARE WORKER**

**SR-KT8 1RG**

**Reports to: Mandy Richmond and Goksel Guner - case managers.**

**Role: Carer**

**Job Hours**

2:1 Carers required 24/7

**Pay Rate**

£14.00 per hour Mon-Fri

£15.50 per hour Sat-Sun

**Background Information**

SR a severe brain injury on 23rd September 2013 and he remains in a low awareness state. He is wheelchair dependent, with no consistent means of communication and is dependent on others for all aspects of his care, mobility and daily living.

**Key responsibilities**

To work with SR in a rehabilitation and support role, enabling the client to lead as fulfilling a life as possible, maximising their physical and psychological well-being and safety at all times giving help and assistance with all activities of daily living. You will build positive relationships with SR, their family and other health and social care. As you will be working in SR home and local community. You should follow the families house rules and be mindful of their need for privacy.

**DUTIES**

**Care and support**

Follow instructions in the care and support plan which has been agreed with SR case manager and/or other professionals. This may include:

- To work on a physiotherapy programme under the Supervision and guidance of a neuro physiotherapist. Refer to physio programme and goals.
- To work on an occupational therapists programme under their guidance to increase both physical and psychological functional ability and using appropriate aids and equipment as taught as apart of daily routines having been assessed as being confident and competent. Refer to OT programme and goals.
- To work on speech/communication under the supervision of a speech therapist and encourage confidence in all communication. Follow any instructions regarding SR ability combined with the therapeutic needs of positioning and postural management. Note this may require specific instructions. Give opportunity for communication. Refer to SLT programme and goals.
- To work with a neuro psychologist implementing taught strategies to increase functioning and manage memory, cognitive problems any difficult behaviours. Refer to neuro psychologist’s goals and strategies for management.
- Generally to work on an active programme of support to include a range of activities. To support recommendations made by the treating therapists.
- Respect the need for confidentiality when SR or their family speak/communicate on matters of a private and personal nature.
- Enable SR to make choices and decisions whenever possible.
- Read and keep updated with all Harrison Associates policies and procedures

**AIDS TO DAILY LIVING**
- Help to keep SR room clean and tidy
- SR washing and ironing to be kept up together. SR to be always well presented and cleanly dressed
- Report any breakages as soon as possible. Report any hazards or health and safety concerns as soon as possible.
- Check all equipment is in safe working order as per risk assessment requirements
- Take SR shopping, outings etc. Keep SR clean and bathed, assist with feeding using techniques, which will be taught to you. Special attention to mouth care.


**Recording and reporting**

Record and report all relevant information including:

- The care and support that you provide and assistance with medicines
- Changes to SR condition or other concerns - you may be one of the few people your client sees regularly over a period of time, it is therefore essential to report any changes or causes for concern to your line manager promptly. You need to be clear about when to seek help and advice in order to keep client safe and promote their well-being.
- Faulty equipment or hazards in the home
- Response to emergencies, accidents and incidents
- Safeguarding matters
- Contact with families or care workers and other professionals
- Keep all information about customers and their families secure and confidential

**Requirements**:

- Follow Care policies, procedures and guidance at all times
- Take part in individual supervision and team meetings
- Attend training activities and appraisal and development meetings
- List client specific training requirements

This list is not exhaustive and you may be asked to carry out additional duties. We will provide full training in line with regulatory requirements.

The ability to respect the privacy of (SR) and their family is fundamental to this position. All matters relating to (SR)’s condition are to be treated as confidential and are not to be discussed with a third party. As this employment is based in the private home it is inevitable that on occasions the support workers will become privy to certain matters relating to the personal life and business affairs of (SR)’s family. These too should be treated with the utmost confidentiality and, whenever possible, the support workers and the team leader should avoid coming into contact with the family’s personal belongings and correspondence.
