Community Rehabilitation Assistant

Come and join our Community Rehabilitation Team in Swindon.
We are looking for a flexible and enthusiastic Rehabilitation Assistant who would relish the opportunity to join our well established inter-disciplinary team and have a desire to utilise and develop their skills in rehabilitation, group work, falls and the treatment and management of long-term conditions.

You will work alongside other experienced Occupational Therapists, Physiotherapists and Rehabilitation Assistants, Nurses, Social Workers, Dietitians, Speech and Language Therapists and Reablement Support staff to deliver rehabilitation in a variety of settings including:
- Home Based Rehabilitation
- Falls Services
- Reablement Service
- Urgent Care Response Service
- Discharge to Assess Unit
- Motor Disorders Service
- Home-First Service
We are looking for a person with some experience as an assistant (Therapy, Rehabilitation, Education, Fitness, Nursing or Care) who is committed to providing the highest standard of intervention to our patients. A training program will be available.
You will need to be a car driver - independent means of travel is essential for this role.
To carry out rehabilitation following an interdisciplinary model in accordance with the CRT philosophy. The post holder will work supervised by therapists, (Occupational Therapists and Physiotherapists) to maintain an efficient day-to-day service.
The postholder will be responsible for own defined caseload following agreed therapy framework
and will share with the therapists the overall therapeutic aim: to enable or restore patients to the
highest level of physical, mental and social independence as possible and support carers in their
role. They will assist in clinical duties, to include the implementation of treatment programmes
for patients.
To assist in the administration and clerical duties of the department such as ordering equipment and supplies enabling smooth running of the department.

Communications and Working Relationships
- To communicate frequently with therapists and where appropriate nursing and medical staff with regard to any change in patient status, behaviour, ability and response to therapeutic intervention.
- To communicate effectively routine information which requires tact or persuasive skills with the patients in an effort to encourage and motivate them towards their therapeutic goals.
- To support patients’ families and partners in their role as carer and communicate any unresolved issues to therapists.
Planning and Organisation
- To be responsible for own time-management around the demands of therapists from a number of different disciplines, by maintaining an efficient diary, whilst ensuring effective delivery of day-to-day services.
Responsibility for client care, treatment and therapy
- To be responsible for own caseload of patients working as part of the interdisciplinary team.
- To assist the therapists with assessment and treatment of patients and, where appropriate, with patients’ continued management including implementation, supervision and progression of therapy interventions whilst under distant supervision from the therapist.
- To work with patients individually and in groups using specific rehabilitation techniques and skills.
- To plan and provide group activities tailored to the client group within agreed framework e.g. balance and safety groups
- To carry out routine and specific tasks with competence as requested by the therapists which assist them in their role.
- To demonstrate a high level of initiative towards all aspects of the work ensuring effective delivery and running of day-to-day services.
- To participate in the assistant training programmes and to take every opportunity to learn and to develop skills, knowledge and competencies which will enhance personal development through life-long learning.
- To participate in team discussion relating to service change and improvement.
- To assist in the teaching of other less experienced assistants and to provide advice and help in prioritising their work.
- To assist in staff training, safe systems of work and departmental procedures.
- To work with volunteers supporting them in the carrying out of specific activities.
