We are looking for an enthusiastic mental health practitioner from a professional background in nursing, Social Work or Occupational Therapy to join an experienced, supportive and established community mental health team.

The role within the community mental health team is to work collaboratively with service users and carers supporting them in the community through their recovery journey. The expectation is t o engage service users creatively to develop care plans including robust risk assessment.

You will need enhanced assessment skills to enable the team to deliver high quality care and treatment plans. Excellent communication skills are essential to enable you to be a role model for others, as well as positively representing the team and the organisation as a whole.

You must also be able to liaise effectively with other services, both internal and external to Southern Health Foundation NHS Trust, retaining person centred care central to clinical decision making to facilitate positive outcomes.

If you are a highly motivated, enthusiastic, flexible, resourceful and resilient individual who is committed to providing care with compassion to service users in their recovery then the Mental Health Community Team is the place to work.

We are able to offer an individualised program of support and professional development to work through with the encouragement from an existing motivated, positive and experienced team. Newly qualified professionals are encouraged to apply.
