We are looking to recruit a full time team-lead for our thriving Health and Wellbeing Coach team in Bridgwater.

The role of Health and Wellbeing Coaches centres on improving patient and community wellbeing - physical, social and emotional - through increasing activity levels. The team of Health Coaches housed at SASP work alongside the PCN Multi Disciplinary Care Teams to provide opportunities, access and confidence around moving more. They will work closely with practice teams to ensure appropriate support is available to people, families and carers to improve the quality of their health. The existing team comprises 5 full-time-equivalent Health and Wellbeing Coaches working at multiple sites across Bridgwater in 1:1 and group coaching settings.

The post holder will work with a diverse range of people and communities and support them to develop their knowledge, skills and confidence to actively look after their own health through positive behaviour change.

The Health and Wellbeing Coaches roles are non-clinical.

**Main Purpose of the Job**

The post holder will be responsible for:

- The management of the 5 Bridgwater Primary Care Network (PCN) Health and Wellbeing Coaches including supporting work planning and training.
- Work with the Multi-Disciplinary Teams within Bridgwater PCN to join up provision and help increase capacity.
- Leading the community consultations within the deprived communities within Bridgwater
- Working in partnership with key organisations to embed physical activity as a positive culture within the PCN.
- Delivering area wide community activity and physical activity programmes appropriate to the target group using an Asset Based Community Development style of delivery where possible.
- Decreasing the numbers of adults that take part in less than 30 minutes of moderate physical activity a week.
- Increasing the numbers of adults exercising to a ‘moderate’ level for a minimum of 150 minutes per week. This will be done through both individual nomination through the Primary Care Network as well as through group activity within the community.
- Increase the number of young people exercising to a ‘moderate’ level for an average of 60 mins at day over the course of a week using individual and group activity.
- Reduce levels of obesity ensuring that physical activity is a key strand in the weight management aspects of the work.

Developing and consolidating partnerships with communities, community groups, voluntary sports clubs, district, town and parish councils in order to drive forward community participation in sport and physical activity. They will work closely with individual communities to facilitate and co-ordinate the delivery of outreach physical activity and lifestyle programmes that encourage healthy lifestyles amongst the community and meet the needs of the target groups within those communities.

**Summary of Responsibilities and Key Duties of the Job**
- Assist the development and strategic implementation of a comprehensive PCN wide community physical activity and healthy lifestyles programme to develop opportunities and support for people to adopt an active and healthy lifestyle within their community including weight management.
- Use the consultation blueprint to work with targeted deprived communities to ensure that an asset-based community development model is used to plan and help deliver interventions to reduce health inequalities through ensuring sport, physical activity, healthy eating and weight management contributes towards the development of community well-being and quality of life for the residents in the area.
- Lead on behalf of the team a specific area of health and wellbeing to gain a PCN wide approach to this both in the community and within the primary care settings within the PCN.
- Manage the 3 Health and Wellbeing Coaches and support their lead areas of health and wellbeing e.g. older people, obesity, children and families etc. to ensure a universal and targeted preventative approach across the PCN.
- Work with other key strategic leads in SASP to work with key partners, agencies and the community to increase the awareness of the benefit of sport, physical activity and healthy lifestyles, and identify needs/issues with regard to the development of sport and physical activity opportunities

Programme delivery and workforce development
- Manage and assist delivery of population level events where appropriate to change attitudes and behaviour around physical activity.
- Optimise the use of facilities such as community halls and schools as a Healthy Community resource to ensure doorstep opportunities for physical activity exist throughout the PCN area.
- Work with SASP to identify social sport opportunities within the area.

Volunteer Development
- Identify community volunteers to sustain programme areas with an emphasis on the deprived communities targeted.
