Clinical or technical To work in collaboration with the MDT to lead and provide an activity programme for patients. Develop a programme of 1:1 and small-group supportive activities and enable patients to participate as appropriate To plan, develop, facilitate and evaluate individual 1:1 and/or group activities and enable patients to participate as appropriate to their physical, cognitive and communicative impairments and personal preferences, with support from the stroke interdisciplinary team. Encourage patients to maintain their personal routines, independence, work and personal interests. Help them keep up their links with family, friends or work.

To be responsible for knowing information pertinent to the patients safety i.e. swallow, communication, weakness, cognition and perception. To plan and implement programmes, adapting and progressing the rehabilitation of patients under guidance from therapists. This will include modifying treatments to take account of changes in the patients physical and mental state with support from the MDT.

Assist in the monitoring of clinical outcomes by accurately recording and reporting data as per agreed service protocols. to identify and report any concerns in regards to the potential abuse/neglect of people using services and carers in line with the Surrey Safeguarding Adults process. To deliver therapeutic interventions that support people to live healthy lives and promote wellbeing promote a healthy lifestyle that includes exercise and the benefits that ensue, with specific reference to keeping active after a stroke/neurological event. Assist in providing an environment in which patients feel safe, welcomed, respected and encouraged, and that facilitate supportive social interaction between patients.

Ensure recreational, craft and other non-medical equipment used by patients is maintained for safety and cleaned in line with relevant policies e.g. infection prevention, food hygiene. Demonstrate the ability to reflect on own communication skills and modify behaviour and practice accordingly. Maintain accurate and legible records of interventions and communication and liaison with other professionals To contribute positively to the effectiveness and efficiency of the Neurorehabilitation team Co-ordinate agreed therapeutic activities, in close collaboration with the MDT, provided by internal or external agencies e.g.

art therapy, music therapy, and liaise with complementary therapy services. Leadership or managerial Manage and prioritise own patient caseload when not leading/facilitating group activities. To ensure supplies of equipment/resources are maintained. To participate in the appraisal system as an appraisee.

To maintain the high standards of the service by contributing towards individual, team and service objectives. To ensure supplies of equipment are maintained and prepare orders when appropriate. To coordinate, organise and support the recruitment of volunteers Ensure that volunteers are appropriately inducted, aware of roles and organise any training co-ordinating with the patient experience team Information and reporting To liaise with the MDT and participate in the clinical decision-making process to ensure planned, coordinated and effective delivery of care. To report back directly to the MDT, particularly any changes in functional abilities, relevant information gained or patients comments linked to treatment.

Ensure activity schemes encompass research findings and evidence to enhance the healing, caring and working environments of the Trust with support of the MDT. Be an active member of the steering group for Healing Arts Programme at ASPH Support the development and deliver effective evaluation of both specific projects and Healing Arts as a whole, engaging with the Healing Arts Group to present outcome. Use this evaluation to inform future events/programmes. Engage with and participate in local and national arts and health networks as appropriate.

Report on issues relating to the activities programme as required. Contribute to information-giving and provide non-clinical support as appropriate. To maintain accurate and legible records of all interventions/contacts and activities provided and communication and liaison with other professionals To participate in relevant meetings May be required to comment on, or contribute to the development of local processes/practices or standard operating procedures. Record any incidents using Trust reporting system.

Undertake and record structured risk assessments for all organised activities in and out of the Bradley Unit, liaising with the appropriate member of the MDT as appropriate to manage risk. Ensure all necessary consents and medical information requirements are obtained for activities and maintain these records safely. Keep records of the referrals, contacts, activity/interventions and outcomes for patients accessing the service and be able to analyse the data to help inform practice Plan activitie
