Support Coordinator

Caring Alternatives are looking for an outgoing and empathic individual with excellent communication skills who enjoys empowering and supporting others.
Being a Support Coordinator is about more than just rota management. You will be responsible for supporting the Service with Growth and ensuring we are always delivering the highest possible standard of care. Liaising with partner agencies for the benefit of the people we support.
We pride ourselves on delivering care that truly meets the needs of the individuals we support, and your role will be fundamental to ensuring this happens every day by matching Customers and Support Workers not only based on availability, but also personality, values, skills and knowledge. You will relish working in a team contributing to the progress of the people we support and contribute to the development of your colleagues.
The role of the Support Coordinator is to work with people using services, Social Care Workers and Healthcare Professionals to ensure seamless, organised and effective support and care.
In Caring Alternatives, the primary duties include contributing to assessment of needs, developing support plans, problem solving as part of the team when problems arise, spending time with service users to monitor the care and support provided and working with the care and support team to assess the effectiveness treatment and the interventions in the support plan, and identify if the desired outcomes are achieved.
**Salary**: £30,000.00-£31,500.00 per year
