Assistant Practitioner

We are currently looking to recruit an enthusiastic and motivated Band 4 Assistant Practitioner to be an integral member of our multi-disciplinary community based team working in Ryedale and across patch to Scarborough if required on occasion. You will be expected to deliver clinical care delegated by a registered professional and contribute to the assessment and evaluation of person centre care plans.
As an Assistant Practitioner you will have the opportunity to work jointly with experienced staff, seeing patients with a range health conditions and will be supported in your work by a robust structure of supervision and in-service training. You will bring experience of working directly with patients in a health care setting, and an enthusiasm for supporting patients to progress in their recovery, empowering them to self-manage their health condition.
To succeed in this role you will need to be passionate about community healthcare provision, able to tackle key priorities with enthusiasm and have the tenacity to deliver in an ever changing environment.
Already established relevant community clinical competencies are desirable for this post.
Working alongside the therapy team and community nursing team undertaking patient home visits to facilitate discharges from hospital, prevent admissions to hospital, carry out assessment & rehabilitation and clinical nursing duties. This could include:
Falls assessments
Assessment for assistive devices for independence and safety at home
Mobility
Management of the deteriorating patient and appropriate escalation
Palliative care
