**Vacancy Announcement**:
We're excited to offer a fantastic opportunity for a full time (37hrs per week) Social Work Assistant (SWA) to join our supportive and friendly Discharge, Assessment and Response Social Work team, based in the heart of vibrant Brighton and Hove. Social Work Assistants provide a vital service to adults with care and support needs and their carers through assessment, support planning and review in accordance with statutory requirements and departmental procedures. Working as part of a team, Social Work Assistant duties are informed by social work knowledge, legislation, statutory guidance and policy and procedures relating to the social care of adults and their carers. They undertake assessments and co-ordinate services to improve outcomes for adults in need of care and support in accordance with our strategic priorities.

**Position Available**:

- 1 Full-Time Position (37 hours per week) undertaking community assessments and reviews for clients who have recently experienced a hospital admission.

- Delivering high-quality and person-centred assessments in acute and community settings with clients after their discharge from hospital. This includes assessing post rehabilitation from health beds and assessing for long-term care at home.

**About the Role**:

- We are seeking a full-time, Social Work Assistant, preferably with some social care experience, but not essential, who is equipped with strong assessment and communication skills.
- You will receive regular 1:1 supervision, have access to Reflective Practice Groups and access to training to support your professional development.
- The role requires knowledge of relevant legislation, including the Care Act and Mental Capacity Act, which is essential. In addition to having knowledge around Safeguarding and the management of risk.
- The role is perfect for those who thrive in a fast-paced, multidisciplinary environment.

**Key Responsibilities**:

- Collaborate with individuals, families, and carers, adhering to statutory legislation and embedding strengths and relationship-based practice in assessments.
- Work collaboratively with the multidisciplinary team in all hospital settings/hospital discharge pathways.
- Demonstrate confidence in identifying and raising safeguarding concerns around vulnerable adults.
- Understand the critical role of information and advice in promoting independence and wellbeing.
