You will work as a key member of the Complex Care at Home Multidisciplinary Team having a clinical role with responsibility for planning, managing and co-ordinating the care of people with highly complex needs and long-term conditions within a defined caseload taking a lead role for people with dementia and low level mental health conditions such as anxiety and depression.

You will be supported by a highly skilled proactive team of registered and non registered health care professionals, who are nurses, physios, occupational therapists, dietitians, health and wellbeing coordinators, adults social care and housing officers. We aim to provide personalised care to the people we support using a What Matters to You approach. You will be a valued member of the MDT where your development, work life balance and job satisfaction will be a top priority.

You will use case management techniques to prevent avoidable deterioration in people’s health and ultimately reducing the occurrence of unplanned hospital admissions.

You will continually develop their advanced skills and competences in and medicines management, clinical assessment, care co-ordination, managing cognitive impairment, supporting self-care, management and leadership skills in line with the Department of Health competency framework for Community Matrons

Provide specialist knowledge and support to other team members

Advise on non-complex mental health issue such as depression and anxiety, understanding the opportunities and processes for referring/signposting for people with more complex mental health problems

You will work as a lead practitioner within a multidisciplinary team, proactively managing a caseload of complex patients living with dementia, long term conditions and/or frailty and at risk of deterioration. Patients will be identified through an agreed case finding approach and from referrals received from primary and acute care teams

Support patients, carers and families to live well with their Dementia to prevent avoidable health deterioration & hospital admission, or unnecessary length of stay through a case management approach.

Be responsible for the assessment of the health and social care needs, gather data and interpret complex clinical information in order to plan care appropriately.
