Social Prescriber

**The Role**

The Social Prescriber Link Worker will work directly with patients, to jointly agree and develop a person-centred plan to help the individual to achieve positive health and wellbeing outcomes by addressing any non-medical or social issues in their life such as, low level ill mental health, debt, or social isolation. Supporting a highly skilled multi-disciplinary network, the post holder will be employed by Involve Community Services but will reach into their given surgeries.

**Responsibilities and Duties**
- Link patients to services across the Health, Social Care and Voluntary sectors of Wokingham borough and surrounding areas
- Work alongside patients to help them achieve their goals through the development of personalised plans
- Empower patients to make the necessary changes to improve wellbeing
- Work closely with GPs, community services, Social Care, voluntary sector organisations, Practice staff and wider multi-disciplinary teams
