We are currently seeking a full-time qualified Nurse, Occupational Therapist or Social Worker to join our Primary Care Mental Health service covering the areas of Hoddesdon, Ware and Hertford.

Successful applicants will facilitate clinics offering a mix of telephone and face to face consultations with service users within their designated GP surgeries. The role includes triaging and signposting service users to access the most appropriate service to support their mental health and wellbeing. This exciting development recognises that placing specialist mental health staff in GP practices will promote early assessment and treatment in a recovery focused way, therefore helping the service user to achieve the best outcomes.

This is a Band 6 mental health practitioner post. You will have a relevant professional qualification (nursing, social work, occupational therapy), and experience of working with service users with mental health needs. The post will generally be based within the primary care setting (GP surgeries). The practitioner will offer primary care mental health liaison support to GP surgeries within a designated hub of surgeries and will have the opportunity to support the development and implementation of this service. The service will provide support to those being referred for support and will also seek to support primary care practitioners in the transfer of service users from secondary care to primary care.

- The role of the Primary Care Mental Health Liaison Practitioner will be to work across the interface between Primary Care and Mental Health Services to work alongside the service manager/s to facilitate mental health assessment in primary care and support GP's regarding decision making in relation to secondary care referrals, support transition of service users from the care of secondary mental health services back to Primary Care and to facilitate re-access where appropriate.
- The post holder will provide consultation, support, education and problem-solving approaches to the Local Primary Care Teams particularly in the area of assessment and management of mental health problems in adults of working age.
- The post holder will run clinics autonomously and will offer telephone and face to face consultations to service users within their GP surgeries.
- The post holder will signpost service users to appropriate services and may occasionally undertake very short-term interventions for clients with mild to moderate mental health difficulties.
- The post holder will refer service users to community mental health teams if the service user meets the threshold for secondary care services.
- The role is to strengthen access and to provide community triage for people who may need mental health services.
