**Role Summary**
- Long-Term Conditions Self-Management (LCSM) is a project that empowers people living with any long-term physical or low-level mental health condition to better manage their condition and to achieve a better quality of life._

The role involves working with the Long-Term Conditions Self-Management Project and Training Coordinator to support individuals to access the My Health, My Way sessions and also any ‘follow-on’ activities.

The post holder will additionally assist in the facilitation of the My Health, My Way workshop led by the Long-Term Conditions Self-Management Project Coordinator.

The post holder will also support a small team of volunteers who assist with the project delivery.

**Key Responsibilities**

1. Support the organisation of the My Health, My Way workshops across the city to include promoting, liaising with, advising and registering new course participants.

2. Facilitate and co-deliver My Health, My Way workshops in a variety of community settings, delivering alongside our volunteer tutors and supporting them as needed.

3. Collate, update and maintain course resources, including setting up of rooms for course delivery ensuring all resources are available for course facilitators.

4. Maintain and create electronic records using a variety of systems to ensure accurate collection of participant data for reporting purposes on a regular basis.

5. Assist the Long-Term Condition Project Coordinator to build a strong network and knowledge of groups and sessions across the city to signpost people to from the workshop and also referrals from individuals, Primary Care Sectors and organisations.

6. Assist in raising the awareness of the project across all sectors of the City and increase understanding of the issues which can affect individuals living with long-term health conditions.

7. Assist with the recruitment and support of volunteer tutors.

8. Undertake supervision, training, and development as appropriate.

9. Undertake other tasks that are appropriate to the role and job description.

10. Maintain confidentiality regarding participant information at all times.

**Person Specification**

**Education**
- Educated to A Level or above
- Level 3 or above in Community Development, Social Work, Education, Health etc. (desirable)

**Knowledge**
- Organisations in the local sector that deliver services relating to health and wellbeing, both statutory and non-statutory
- Local knowledge of the Plymouth area and the organisations that operate within it.
- Understanding participants differing needs (e.g. mental health, learning disabilities, long-term health conditions) and how to adapt sessions to ensure effective communication and inclusivity.
- Understanding the needs of individuals living with long term health conditions and their likely risk factors including physical health, mental health, social isolation and loneliness.
- Understanding of the wider Adult Social Care system and Information and Advice.

**Skills**
- Excellent general IT and database skills
- Workshop facilitation
- Excellent verbal and written communications skills
- Effective writing skills with the ability to draft correspondence
- Able to be creative within the scope of the role
- A ‘people person’ able to work in a non-judgemental and non-discriminatory way.
- The ability to prioritise tasks, to identify and work to deadlines and to manage time effectively under own initiative.
- Ability to work in co-operation with a committed hard-working staff team

**Experience**
- Working with groups of people who face barriers to healthcare/health inequalities
- Course/Programme/Intervention delivery within a health & social care setting or related field.
- Experience of planning and delivering/or assisting activity sessions.

**Other**
- Full driving licence

**Job Types**: Part-time, Permanent
