An opportunity has arisen for both full time and part time Specialist Dietitians to work as an integral part of the Frailty Intervention Team.

This role is an exciting opportunity for you to be part of developing a new model of care for the Morecambe Bay population living with frailty.

The Frailty Intervention Team pathway works on a “early intervention, better outcomes” philosophy with a focus to returning patients to their usual place of residence on the same day as presenting in the Emergency Department where appropriate.

The Frailty Intervention Team is a growing and developing team which with successful recruitment will become a 7-day service. It will provide the successful applicants autonomy and the opportunity to shape the service.

As the service develops, it will continue to build on strong links with our community services and establish formal links with pre-hospital urgent and emergency care services to offer direct referrals. There are also future plans to develop a dedicated Frailty assessment area in our Royal Lancaster Hospital to support the service as it grows.

This is an exciting time for our services and we hope you will want to play a large part in developing a gold standard service which will achieve our aim of delivering excellent care in the Morecambe Bay area for our Frail and older patients.

The successful applicants will be an integral member of the newly created Frailty Intervention Team based in the Royal Lancaster Infirmary. The aim of this team to to prevent admissions and support discharges in a timely way for our frail and older patients thus minimising their length of stay in hospital.

You should be a HCPC registered dietitian. The post will be based at Royal Lancaster Infirmary however some of the role will be working with patients in the Community.

You will work independently as an autonomous practitioner providing flexible and patient centred care.

Provides support in the specified clinical area with specialist knowledge, skills and experience to advise and support staff on managing complex clinical issues for the Frailty Intervention Team.

To be responsible for managing a caseload of patients with a range of frequently complex conditions, appropriate to specialist knowledge and skill level and experience.

The Dietetic department is approved for student training for Placements 1; 2 and 3 dietetic students. We have excellent IT facilities and are committed to personal development and training.

University Hospitals of Morecambe Bay NHS Trust operate from three main hospitals - Furness General Hospital (FGH) in Barrow, the Royal Lancaster Infirmary (RLI), and Westmorland General Hospital (WGH) in Kendal, as well as a number of community health care premises including Millom Hospital and GP Practice, Queen Victoria Hospital in Morecambe, and Ulverston Community Health Centre.

FGH and the RLI have a range of General Hospital services, with full Emergency Departments, Critical/Coronary Care units and various Consultant-led services.

WGH provides a range of General Hospital services, together with an Urgent Treatment Centre, that can help with a range of non-life threatening conditions such as broken bones and minor illnesses.

All three main hospitals provide a range of planned care including outpatients, diagnostics, therapies, day case and inpatient surgery. In addition, a range of local outreach services and diagnostics are provided from community facilities across Morecambe Bay.
