Domiciliary Home Care Planner

Adferiad Recovery operates domiciliary support services including domiciliary home care, children’s services, palliative care and respite provision for Carers. In line with our commitment to principles of empowerment, self-management, wellbeing and recovery and ensuring compliance with internal and external quality standards inspection and regulation (including CIW).
The position of Care Planner responsible for:
- The completion of processing referrals from point of receipt to the delivery of support through the creation of person-centred support delivery plans (including risk assessments), reviewing of care & support packages, while liaising closely with health and social care professionals as and when required.
- Providing support to the Service Manager and Registered Manager, in monitoring and evaluating staff performance though the completion of supervisions and observations and the provision of ongoing support to support staff.
About this job:
The role is for 37.5 hours per week, working flexibly as part of the Care Office Team in the delivery of flexible, tailored packages of care and support promoting individual’s wellbeing, rights and independence.
