- We currently have positions in our Integrated Urgent Care Team and also our Integrated Urgent Care hub, soon to be part of our new assessment centre. It is an exciting time to come and work for us as you will be joining the team at a time of transformation and great investment into services for adults and social work practice.
- Assessors within the Integrated Urgent Care Team will take the lead on supporting adults to live well at home. You will be part of a friendly and supportive team and your work will include managing a caseload, responding to urgent request for Care Act Assessments, working closely with our health colleagues to get the best outcomes for service users. You will also be responsible for completing Support Plans, Mental Capacity Assessments and Best Interest Decisions and signposting to internal and external services where necessary to enhance wellbeing and independence.
- Assessors within the Integrated Care hub will be responsible for completing triages of need either in person or on the phone with service users at the beginning of their social care journey. Triages are completed on behalf of health and social care within our integrated team. Empowerment and prevention are key as we support adults in Tameside to live at home for longer as well as completing necessary signposting to internal and external services where required to enhance wellbeing and independence. You will have excellent communication skills and have the ability to build effective professional relationships with other disciplines within our integrated team and also with other local health and social care services to enable a coordinated response to individuals and families. As part of the integrated Urgent Care hub you will be expected to work as part of a shift rota which covers weekends and evening shifts. Cover is required between 7:30am until 10pm each day and split into Early and late shifts. This Includes bank holidays.

- Experience of working with adults and previous employment in a similar position would be desirable.
- It is essential that you hold a current valid driving licence and have access to a vehicle for work use.
