Healthwest is a Primary Care Network made up of 4 practices (Pembroke Road Surgery, The Family Practice, The Students’ Health Service and Whiteladies Health Centre). Together the surgeries provide care to over 70,000 patients in our local area.

We are looking for a highly motivated, flexible, reliable and friendly individual who is passionate about patient care to join our community based team and enhance the proactive care that we offer to our patients.

You will work as an active member of the multidisciplinary team supported by our Community Advanced Nurse Practitioner and practice based clinicians. You will deliver care within the boundaries of your role, focusing on identification and monitoring of long-term conditions and the enhancement of patient access to care. To enable relationships to develop with the individual nursing teams and the surgeries your timetable will include working in the surgeries 1 or 2 days per week.

**Roles and responsibilities**

You will have an understanding of the nursing process and be able to assess, plan, implement and evaluate programmes of care to improve health and well-being and meet the healthcare needs of individuals and groups.

Prioritise health problems and intervene appropriately to assist the patient in complex scenarios.

Demonstrate an ability to work under pressure, prioritising workload effectively.

Demonstrate an ability to use a problem-solving approach within limits of your competence.

Demonstrate an ability to provide holistic patient centred care

Work in a solution
- focused way as part of a multi-disciplinary team.

Promote holistic healthcare and work in partnership with other agencies.

Access and participate in supervision.

Maintain an accurate diary in accordance with lone working policy and adhere to mobile working guidance.

Identify and take appropriate action/make appropriate referrals in any situation where there may be vulnerability, safeguarding or health concerns

**Person Specification**

Qualifications and Experience

Registered Nursing Associate on the NMC register.

Nursing Associate foundation degree qualification.

Understanding of the scope of the role of the Nursing Associate in context of the nursing and interdisciplinary team.

Understands and acts in line with NMC professional standards for practice contained within The Code.

Knowledge of when to seek advice and escalate to the appropriate professional for expert help and advice.

Understand requirements for NMC professional Revalidation

Experience of working with vulnerable patients

Skills and Competencies

Computer literate with an ability to use the required GP clinical systems and Microsoft office packages **(desirable)**

Has attention to detail, able to work accurately, identifying errors quickly and easily

Ensure clear, accurate and up to date recording of information on EMIS as per local record keeping policy.

Has a planned and organised approach with an ability to prioritise their own workload to meet strict deadlines

Excellent communication skills, verbal and written, with the ability to adjust communication style and content to suit the audience

An excellent understanding of data protection and confidentiality issues

**General**

The postholder will:
