**Discharge to Assess care home beds (Monday to Friday)**

**Band 4**

The invoices will need to be made direct to the CCG for D2A. Part time also accepted. Monday to Friday.
- The role is countrywide across Buckinghamshire and wherever possible, care homes will be allocated to reduce travel however this may not always be possible as depends where the patients are.
- Therapists will assess the patient in the care home for therapy needs and functional requirements, care needs and have close liaisons with the social worker in order to agree discharge plans for the patient to be discharged home. Band 4s will follow thetherapy plans et out by the therapists. Therapists and band 4s will undertake follow up checks at home if and when required.
- The therapists must have experience and sound judgement around discharge planning, MDT working, be autonomous and able to work as a lone worker in the community. The band 4 must have experience in rehabilitation.
- We will provide the locum with a laptop so they can work in the care homes and from home, only needing to go into base to collect supplies and PPE as required.
- Must have a car
- Must be COVID 19 vaccinated as working in care homes
