Are you passionate about palliative care? Do you want to work with an enthusiastic team developing your palliative care skills? Hampshire Hospitals Palliative Care team is looking for a Band 6 Palliative Care Clinical Nurse Specialist to join their team based predominantly in the Hospital in Winchester.

The Specialist palliative Care band 6 CNS post holder will practise autonomously, carrying a caseload of patients, supporting the wider team in the delivery of a seven day, twenty-four hour service for which you are responsible for or have been delegated the responsibility, this includes:

- High standards of Professional Practice to ensure quality and safety of patient care, experience and the patient journey
- To develop specialised clinical practice, knowledge, skills and experience in palliative care
- Contribute to the delivery of education in palliative and end of life care
- Attend site specific MDT’s and Morbidity and mortality meetings
- Leading by example
- Supporting and contributing to the team in ensuring that clinical services are delivered to a high quality of care and that all compliance requirements such as CQC and contractual requirements are achieved

Establish local and national networks, linking with other practitioners in specialist area in order to share/learn from examples of good practice. Work with relevant clinical groups at cancer network level in order to deliver local and national objectives for the specialist area

Responsibility for patient/client care
- To develop and deliver effective, programmes of care, recording, documenting and maintaining these within trust policies.
- Provide clinical advice around Palliative and end of life care
