Currently, we cover the Boroughs of Epsom and Ewell, Reigate and Banstead, Ashtead, Leatherhead and surrounding areas.

**The Care Coordinator is responsible for**:

- In charge of the weekly rosters, including seeking cover when carers are absent.
- In charge of scheduling for the following week.
- Good planning and control are strictly required.
- Willing to cover the calls in emergency situation
- Care Planning and Risk assessments
- Participating in the Client reviews
- Conducting Care workers supervision
- Participating in Weekly OnCall duties and responsibilities

The first point of contact for new and existing clients, with regards to care visits and minor requests, or directing them to the right person/department.

Ensure that all booked care hours are filled.
Work with the Care Manager and colleagues to implement new care packages on the weekly roster, including identifying the right carers.

Ensuring all shifts are correctly in the system and confirmed every week, before the payroll and invoicing run.
- On-call Duty as required and discussed.
- Undertaking client assessments and reviews from time to time - as required by the Care Manager.
- To ensure continuity of care provision through effective contingency plans

Ensuring all duties are in compliance with CQC regulations, maintaining an understanding of regulations & updating knowledge as required.

Undertake care calls as agreed with the Line Manager.

The ideal applicant will have experience in Domiciliary Care Agency with at least 2 years of experience as a Care Coordinator or Scheduler or a Senior Domiciliary Care Worker, preferably with NVQ level 2, NVQ level 3 in Health & Social Care.
