MAIN DUTIES/RESPONSIBILITIES Key responsibilities and duties of the post holder are: To manage referrals coming in the service in liaison with service administration staff. To make contact with people referred to the service within 2 weeks of their initial contact. To support planning of and participate in post diagnostic group programmes and stand-alone education sessions. (There will be a requirement for occasional working at a weekend or evening to support the above).

To work with individuals with a diagnosis of Mild Cognitive Impairment or a Dementia and their supporters/carers, in the development of a support plan that is enabling and empowering and allows them to make choices that are meaningful for them. To provide information regarding services and available support and where needed to help with access to these. To maintain accurate and timely electronic and written records in relation to contact with users of the service in accordance with organisational policy and in best practice around data protection and governance principles. When necessary seek support and guidance from other professionals within the service.

Provide support to people approaching the service for guidance and signposting around obtaining support or diagnosis. To promote principles around living well with Dementia or Mild Cognitive Impairment and in a person centred manner. Participate in service development and planning Maintain health and safety of self and others through adhering to team and organisational guidance and policy. Participate in regular supervision and annual appraisal with line manager or designated other team manager.

Participate in the evaluation of services provided and ensure that service users are given the opportunity to evaluate the service they have received. Ensure that service users are reviewed in line with service requirements. To use equipment, issued to post holder in line with their role, in a safe and appropriate manner and ensure that security of ssuch items is maintained. 6.

COMMUNICATIONS AND RELATIONSHIPS To work in a cohesive way with colleagues to ensure continuity of support for service users and service objectives are achieved. To create and maintain relationships across organisations that support service users To act at all times in such a way as to support a positive reputation of the service and instill confidence in users of the service.
