**Are you passionate about providing high quality patient care? The North & East Community Nursing teams would like you to support them in delivering high quality care and support to people in their own homes.**

This is an exciting and fulfilling role and you will need to be able to demonstrate a real enthusiasm for nursing people in the community as well as a desire for on-going professional development.

You should also have an understanding of the changing climate of care delivery in the community with the ability to travel independently across the area to fulfil the requirements being essential.

**Main duties of the job**:
The role of the Nursing Associate is to work with the Registered Community Nurses and other members of the multi-professional team, providing and monitoring care and contributing to the ongoing assessment,
as agreed with supervising clinician(s) and in line with the service pathways. They will monitor the condition and health needs of people within their care on a continual basis in partnership with the multi professional team.

This role will support the team with coordinating and delegating work to the Health Care Assistants. Support the team with organising other resources such as equipment. Support the team by providing supervision, teaching and coaching. To review patients on the case load and adjust care plans.
