Chelsea and Westminster Hospital NHS Foundation Trust has an exciting opportunity for nurses and Allied Health Professionals keen to extend their skills in diabetes and endocrinology.

The Diabetes Specialist Practitioner team provides expertise in all aspects of diabetes care provision; involving the expert management of diabetic inpatients and outpatients as well as specialist groups such as antenatal diabetes. This post is designed to recruit staff from a variety of disciplines who have an interest and some experience in diabetes, with a view to providing training and supervision to build independent practitioners of the future. This will be in line with the pan London competency framework. We are seeking current band 6 nurses, pharmacists, dieticians or therapists who are motivated, enthusiastic and keen to develop their skills in diabetes management.

This will be in line with the pan London competency framework. We are seeking current band 6 nurses, who are motivated, enthusiastic and keen to develop their skills in diabetes management.

We have recently completed a £30m expansion of our adult and neonatal critical care facilities at Chelsea and Westminster and are planning an ambitious £60m development of an Ambulatory Diagnostics Centre at West Middlesex.

Clinical/Direct Patient Care
- Work both autonomously and as part of the wider MDT in the management of a caseload of patients.
- Maintain effective communication with patients, carers and professionals to ensure optimal service delivery.
- Provide expert knowledge of diabetes and work within an agreed job plan to meet service delivery needs.
- Become an expert in the use of insulin pump therapy, insulin administration, diabetes therapies as well as the use of blood glucose monitoring equipment.
- Complete competencies in The Integrated career Competency Framework for Diabetes Nursing
- Participate in MDT led clinics and support clinics, to include ordering, performing and interpreting relevant clinical examinations as well as providing education and counselling for patients.
- Maintain awareness of own professional limitations, act within own scope of practice and agreed guidelines and protocols. Make use of professional judgement when managing complex or unpredictable situations.
- Provide health promotion and counselling to diabetic patients to reduce the risk of diabetes related complications.
- Support patients to use technology solutions to manage their diabetes
- Plan and conduct appropriate follow up of diabetes patients to ensure safe and effective diabetes care delivery.
- Maintain accurate, concise and contemporaneous records of patient care.
- Communicate in a clear and effective manner with patients, carers and colleagues.
- Empower patients to make decisions regarding their care and advocate for them within the wider MDT.

Leadership and Collaborative Practice
- Proactively provide specialist diabetes advice, guidance and support to the wider hospital MDT to enable best practice in care and treatment of people who have diabetes.
- Work collaboratively with colleagues, patients and families to seek their views and to develop and improve diabetes practice and the diabetes service.
