This is a rare opportunity to join the Ferndown and West Moors Older Persons Community Mental Health Team at Oakley House in Wimborne.

This multi-disciplinary team provides recovery focused treatment plans for those people who are 65 and over with SMI or who have organic mental ill health including Alzheimer's disease.

The team is supportive and proactive both to service users and colleagues alike. We have a good record for retention of staff and are always looking for ways to improve our services and maintain high standards of care for those we serve.

This is a challenging job, but if you’re ready for this then we are looking for someone who is reliable, committed and motivated with excellent communication, organisational skills and the ability to prioritise. We offer our team structured management supervision and a supportive team environment. In addition we commit to facilitating you with your own professional development.

The team adapted its working practices to deal with the COVID pandemic utilising remote technologies, this will allow the applicant to do some working from home. However the expectation is that staff will be mostly office base. There is good peer support in the team in addition to the wellbeing options offered by Dorset Healthcare

- To provide a comprehensive, accessible and responsive mental health service to persons over 65 and their families within a defined locality.
- To manage a caseload of patients with a mixture of functional and organic disorders
- To continually develop clinical, team and service evaluation and delivery of service.
- To contribute to effective multi-disciplinary working within the community mental health service and to build up effective liaison with community, primary, and other secondary and tertiary services/agencies within the locality to provide holistic care based on the individual’s need and service development.

The team office is in Wimborne and the post holder will travel from there to visit patients in the Ferndown and West Moors locality. The post holder will need to have the ability to travel by car, with business insurance, as they will be required to assess and monitor patients in their own homes and within other community or inpatient settings.
