The Early Supported Discharge Stroke Therapy Team provides ongoing support and rehabilitation to people with stroke on discharge from hospital.

An exciting opportunity has arisen within the Stroke Early Supported Discharge Team for an Administration coordinator. The role will involve providing administrative support the Stroke multidisciplinary therapy team team.The role would be based at Aintree University Hospital but there may be occasional where cover is required at Broadgreen Hospital.

Support the team in the collation of data required Nationally to monitor and support service delivery.

To provide administrative support to the ESD stroke team to ensure safe and effective delivery of patient care

To act as a point of contact for all forms of communication for health and social care professionals, patients and carers thereby supporting the delivery of a high quality service to patients and staff

Liverpool University Hospitals NHS Foundation Trust was created on 1 October 2019 following the merger of two adult acute Trusts, Aintree University Hospital NHS Foundation Trust and the Royal Liverpool and Broadgreen University Hospitals NHS Trust.

To provide comprehensive administrative support to the ESD MDT

To record and analyse agreed data for the ESD team in accordance with Trust and national guidance

To collate patient sensitive data to support accurate and timely reports

To input accurate and timely data into databases and systems

To be responsible for coordinating key team meetings with appropriate staff ensuring outcomes are notified to the appropriate staff within the agreed time frame

To ensure the ESD service is organised in accordance with team processes

To collate and distribute information using appropriate mean of communication including electronic means

To ensure relevant documents and resources are available for the team

To obtain all necessary documentation (including case notes, results and statistics) to assist in reporting and conducting audits

To ensure communication regarding patient care is prompt

Deal with correspondence, telephone and personal enquiries relating to appointments, cancellations and information.

Organise activity in line with Trust policy

**To communicate with all members of the ESD team**: therapists, nurses, admin staff & staff from other relevant department/bodies

Coordinate aspects of meetings related to the ESD team maintaining a diary, conforming staff availability, booking rooms, constructing agendas, taking accurate minutes and distributing notes ensuring outcomes are documented

Maintaining filing and information systems including archiving of patient records as appropriate

Providing administrative support to the ESD team to include organising meetings, contacting patients, regarding appointments, supporting induction training and study days internally and externally and conferences.

Prepare letters on behalf of the ESD team
