Job Summary:
We are seeking a professional and skilled part-time (with scope for full time) Care Coordinator who excels in efficiency and is able to deal with multiple tasks associated with the provision of care. Although this role is part-time (3 days per week) there is scope for development into a full-time role. Having a UK driver’s license and your own transport is essential for this role.

**Duties**:

- Advise and direct care staff in their role
- Meet and arrange care for new clients, making assessments of care needs and liaising with healthcare professionals
- Conduct reviews of clients and do staff supervisions
- Take phone calls, arrange staff rotas and training sessions
- Maintain industry and CQC standards of care provision
- Deal with emergency care situations in the community

**Experience**:

- NVQ Level 3 or above or equivalent qualification
- Experience in care coordination

This role is part-time for 3 days per week but can be developed into a full-time role. This can be discussed at interview. Pay is: 26k to 30k Pro Rata.
