We are seeking a motivated and dynamic Band 4, Qualified Nursing Associate to work within our Primary Home Care Team - North. You will be based at Twinwoods Health Resource Centre (Clapham), however there is an expectation that staff will cover across the bases if needed. This is an excellent opportunity to be one of the first Nursing Associates to join our Community Nursing Teams. To develop your knowledge and skills within the community nursing setting, with the possibility of being supported to complete your Nursing qualification. You will have the opportunity to be involved in facilitating self-care for patients with long term conditions, health promotion and innovative activities and projects that enhance patient service delivery and staff wellbeing.

You must have a driving license and have access to a vehicle in order to carry out your duties. You will be required to work a shift pattern covering 7 days a week from 08:00-21:00.

To provide, supervise, and monitor care and treatment to an identified group of patients under the direction of Registered Nurse.

Take responsibility for the development and delivery of evidence based practice, ensuring that it supports and reflects service user individual needs, Trust objectives and professional guidance.

Contribute to the development of high quality evidence based in-patient services in a changing health care environment.

Contribute towards ensuring that the service functions as an integral part of the Directorate’s services.

Effective communication with all relevant personnel both internal and external to the Trust.

To practice within professional guidelines and promote professional practice in clinical area.

To ensure continuous personal development through the utilisation of regular supervision, annual appraisal and training where appropriate.
