Senior Care Assistant

We have both full time and part time positions available. FULLY FLEXIBLE
**Brief description of duties**
To act as shift senior and fully complete the daily delegation board on commencement of shift, ensuring all required tasks and appointments are allocated and achieved as scheduled. Administer medications as per prescription. Must hold medication qulaification or working towards.
To provide support and direction to the team, in collaboration, during a span of duty, ensuring safe and effective consistent care delivery to service users as per care plans.
Effective use of allocated daily resource maximising opportunities for service user engagement and community presence.
Work flexibly across a 7 day rota to meet the needs of the service users. This includes days nights weekends and bank holidays.
Act in a professional and non-discriminatory/non-judgemental manner and as a positive role model at all times.
Adopt a senior presence and responsibility at all times when on duty.
Act as a positive role model at all times modelling expected professional conduct and therapeutic, empowering attitude with service users and staff.
Maintain appropriate staff boundaries commensurate with a senior staff role.
Work in a manner, and direct the work of others, to minimise risk of infection outbreak and adhere to all guidance re infection control processes including hand hygiene practices.
Undertake additional tasks as designated to senior staff including medication management, health and safety audits etc.
To protect the rights of service users.
To ensure service users are safeguarded from potential or actual harm.
Evidence a commitment to clinical governance, quality improvement
Have an awareness of the Key lines of enquiry and CQC regulation framework and how this is evidenced in practice.
To take the lead with visitors and other professionals
To adhere, and monitor adherence, to Cedar House Care Home policy and procedures in place at all times.

**Communication**
- Communicate in a clear, concise and complete way including any explanation necessary to effect appropriate response from others.
- To maintain confidentiality in all areas of communication, written, verbal and electronic both internal and externally to the care home and its staff and users.
- Uses appropriate language and communicates at a level that is appropriate to the person.
- Cascades information correctly and in a timely manner.
- Escalates any concerns via the correct route.
- Ensure due regard is given to service user rights when dealing with sensitive information and disclosure of same on a need to know basis.
- To elicit service user consent to share information about or on behalf of service users when required to.
- To give a daily handover to managers of any issues and concerns that have been observed during the previous 24 hours.
- To ensure effective communication across all parties and that handover of relevant service user information is given to managers to ensure effective care delivery at all times.
- To attend relevant meetings as scheduled and contribute to same.
- To liaise with families, carers and other professionals as and when required and in a professional manner, ensuring information sharing is relevant and necessary at all times.
- To contribute to or provide reports and data as and when required
- CPA, medication review etc.
- To ensure effective entries in progress notes and detailed handover information is documented each shift.
- To ensure entries made in the communication book are relevant and suitable.
- Ability to seek out and weigh up information to make decisions on a day to day basis.
- Give positive regard to individual service user and staff contributions and qualities
**Care delivery**
- Work in a person-centred manner at all times.
- Support service users to be fully involved in designing their care by engagement in the care plans in place.
- Have an overview of all service user needs and care plans and refer to same if advising staff re interventions etc.
- Ensure information is available, explained and presented in a format that Service users can understand to enable them to be aware of information collected about them and on their behalf.
- Allow time and explanation to ensure service users feel involved and informed in care planning and seek their views when developing care plans.
- To support service users to appointments and activities as indicated.
- Develop therapeutic, valued and meaningful professional relationships with service users to maximise opportunity for informed decision making at all times.
- To promote safe and effective care delivery, protecting service users from actual or potential harm; Ensuring safeguarding issues are reported as per local policies.
