**Job summary**

Hatfield Primary Care Network is a group of 4 friendly and supportive GP practices looking after 52,000 patients in the Hatfield area. We are expanding our team and eager to recruit an experienced paramedic.

The PCN advanced paramedic practitioner at Hatfield PCN will be a registered paramedic with post graduate qualifications. This role will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of their care. They will demonstrate safe, clinical decision-making and expert care for patients within the PCN.

This exciting role will help to establish new and innovative ways of working to benefit our patients and help our surgeries deliver high quality, responsive care to our patients. The post holder must own their own vehicle as home visits and care home visits will be an expectation.

The **main responsibilities** of the role include:

- To urgently respond to referrals and provide assessment, diagnosis and treatment at first point of contact by attending to patients, according to patients’ needs either in the practice, care homes or patients' homes;
- To support and continue the development of an on the day emergency patient service in the PCN;
- To accurately triage and prioritise patients, including in emergency situations and telephone triage;
- To demonstrate the use of a variety of techniques to elicit the history of an event or illness, including past medical and drug history;
- To evaluate clinical information from examination and history taking, and initiate appropriate treatment and/or referral;
- To review the effectiveness of the treatment provided, making changes where necessary to improve patient outcomes;
- To maintain accurate clinical records in an efficient manner;
**Person specification**

The role will offer you the opportunity to build on your experience of working effectively within a multi-disciplinary healthcare team, as you will work alongside fellow professionals within the practice, in support of one another.

You will have the following skills and experience:
1. Be a registered paramedic with Health & Care Professions Council (HCPC) registration;
2. Educated to degree/diploma level in Paramedicine or equivalent experience;
3. Evidence of completion of two-year ‘Consolidation of Learning’ period as a newly qualified paramedic;
4. A minimum of a further three years’ experience working as a band 6 (or equivalent) paramedic;
5. Evidence of working towards developing masters’ level or equivalent capability in paramedic areas of practice and, within six months of the commencement to have completed and been signed off formally within the clinical pillar competencies of the paramedic FCP/AP roadmap to practice;
6. Have completed ENP/ ECP minor injuries and minor illness course level 6;
7. To be able to effectively assess, plan, treat and refer patients appropriately and evaluate patient care confidently;
8. Have experience of working to protocols / guidelines and patient groups / patient specific directives;
9. To exhibit excellent communication skills when dealing with clients: non-verbal and verbal skills required;
10. Have the ability to make clear decisions with confidence and communicate these effectively;
11. To work within the parameters within skills and knowledge, to recognise the limitations of practice and to work within the boundaries of training and capability, according to the HCPC code of conduct;
12. Experience of autonomous practice working and multi-professional team working;
13. Can demonstrate a broad and sound knowledge of complex and common conditions in primary care in both adults and children;
