We have a rare vacancy in our team for a qualified and experienced ACP this is due to expansion of the service. Hertfordshire Community Trusts (HCT) Hospital at Home service was set up during the first wave of the pandemic in March 2020. The Service is always evolving as it responds to changing priorities within the community population or to alleviate particular pressures on hospitals. We are always seeking to improve and collaborate with others and would love you to join us on our journey.

The focus is to keep patients out of hospital so if their condition can be managed safely and appropriately in the Community and it is their wish to remain at home we try to facilitate this.

The Service runs 7 days a week from 8am-8pm and is based in the Referral Hub at Robertson House, Stevenage.

Referrals come from Community GPs, A+E, acute wards, out of hours and other allied healthcare professionals such as paramedics or those working within HCT eg district nurses, physio's or care homes. All referrals have to be triaged by a member of the team and once it is felt we can add value to a person's care, a management plan is formulated involving a multidisciplinary team including GPS, nurses, pharmacists, physios and other services such as the community respiratory team or palliative care team. We aim to keep patients under the Hospital at Home team for no more than 2 weeks but some do stay longer and care is individualised according to a patient's needs.
