**Care Leaver - Personal Advisor**

Hampshire County Council's Care Leavers' teams are committed to improving outcomes for young people when they leave our care. As corporate parents, we want our young people to grow into confident adults and support them in making the transition into adulthoodand independent living; we know that success will look different for everyone.

Working with young people aged 16-25 in their transition to adulthood and independent living, you will have considerable experience of working with young people, and of liaison with other agencies in putting together support plans. You will demonstrate theability to build relationships with young people, face difficult issues and deal with them sensitively, and to be a team player.

Please note that travel is required as part of your role, as you will need to visit our young people at their home address to ensure they are managing with their transition into adulthood and independent living. Generally, this means that you will requirea full driving licence and access to a car, but reasonable adjustments to this requirement will be considered to cater for a declared disability.

**Role Responsibilities**:

- Work with Care Leavers (18+).
- Additional Personal Adviser support for 16-18 year olds.
- Assessment of needs and Pathway Plans for care leavers.
- Supporting young people with specific issues, signposting to specific agencies, as required.
- Contribute to the prioritising of areas of work.
- Supporting Team Manager and Senior Practitioner to develop and deliver the service plan.
- Working as part of a team and utilising resources to ensure that an effective and efficient service is provided to care leavers and their communities within the team area.
- Working in partnership with other agencies and across departments and liaising with parents and carers to support care leavers.
- Delivering services that accommodate diversity and reflect best practice in equalities.
- Complying with appropriate policies, codes of practice and legislation, ensuring safety of self and others.
- Visiting care leavers and maintaining contact with them, as per their needs.
- Ensuring that care leavers have been advised of their rights as per the local offer.

**You will need**:

- Experience of working with young people.
- Experience of planning, delivering and evaluating work with young people.
- Experience of successfully co-operating and liaising with other agencies.
