We are recruiting for a **Support Mentor** at our Penrose Complex Needs Hub, based in the Royal Borough of Kensington.

**THE SERVICE**

The SIG: Penrose Complex Needs Hub is commissioned to provide safe, accommodation-based housing and relational support services for people who are deemed to be socially excluded.

The Complex Needs Hub is a service designed with a Supportive Relational Model, this aims to deliver support interventions with the residents and participants needs and aspirations at its heart.

We foster the following elements within the model; strength/asset based approaches, trauma informed practice, psychologically informed environments and co-production.

We understand that many of those accessing our service have experienced multiple disadvantage and adverse experiences. By working within this model in a trusting, flexible tolerant and resilient way we aim to assist and equip residents and participants to achieve their potential for independent living through a time of continued change which will bring with it challenges and opportunities.

The needs of service users may include but is not exclusive to:

- Single homeless people with support needs
- Rough sleepers or former rough sleepers
- Offenders or ex-offenders
- Refugees
- People with substance misuse needs
- People with multiple needs, including mental health needs
- People with challenging behaviour(s)

**THE ROLE**

The main purpose of the Support Mentor role is to develop and maintain trust, supporting residents to build capacity for positive interactions and healthy relationship capital that will enable sustainable change and enhanced personal wellbeing. Working alongside the wider Complex Hub team and other local agencies in the Borough the Support Mentors will ensure that individualised support is available to residents and that they are delivered within an enabling environment and relational model to build trust with those we support.

You will work with residents providing 1-1 support as well as other key engagement activities, supporting them to further develop their talents and self-belief whilst managing their mental health and wellbeing.

You will support, inspire, and motivate residents to develop the required skills to build confidence and move into safe, successful independent living within the community. You will provide robust risk management, sharing information with relevant partners to help keep your residents safe.

**OTHER** **KEY RESPONSIBILITIES INCLUDE**:

- To work directly with service users in providing a high-quality delivery of service using appropriate planning and monitoring tools
- To ensure that the service users are empowered at every reasonable opportunity to make decisions and take control over their life and service unless this is clearly detrimental to their well-being
- To create a trauma informed & Psychologically Informed Environment (PIE) so that the service users can move their life forward looking for real community connections, friends, hobbies, interests, work and being creative and challenging about the support provided
- To be creative in involving the service users in meetings which ensures they remain at the centre of the discussion and make choices and decisions
- To support service users to achieve their goals, in line with their support plan
- As part of service users support plan, to liaise with external professionals, e.g., GP’s, probation officer, Care Coordinators, Housing workers etc.
- Responsible for assessing psychological needs of service users and helping to implement appropriate psychological interventions
- To be responsive to the needs of service users and contributing to designing appropriate risk assessments and support plans
- To deliver or support the delivery of group sessions and interventions alongside the support of the Activities Coordinator
- Act as named Support Mentor for service users as designated by the Service Management Team
- Plan and co-ordinate individual support packages for the service user, liaising with the appropriate agencies and care professionals to ensure successful delivery of the plans
- Constantly ensure there is genuine engagement with the service users using appropriate methodologies and interventions to encourage and support their engagement and progress towards greater independence and community integration
- Work alongside the Resettlement Coordinators to identify alongside Residents appropriate and aspirational move-on. With a core focus on private rented accommodation and realistic housing options, this may be out of borough. Working to dispel the stigma aroun
