Elderly Care Facilitator

15-20 hours p/w
- Fixed Term possibly leading to permanent position
- £9.34-£11.50 p/h

Are you self motivated, driven and have a passion for elderly care? This could be the role for you. We are seeking an Elderly Care Facilitator, linking health and social care together to deliver all round care to our elderly population.

**Main duties of the job**:

- To provide administrative support to all members of the Practice team, and patients centred care of those aged 75 and over.
- To run the Elderly Care Coffee Club, organising activities and patient transport where necessary
- To follow up patients identified as requiring social or medical input and arrange appropriate consultation with clinician.
- Ensure collection and maintenance of statistical information required for regular and ad hoc reporting.
- To assist in seasonal and special projects as requested eg carers week, flu vaccinations, etc.
- Monitoring, using clinical system, newly identified dementia patients and subsequent carers.
- Maintain the Practice Carers register and work with relevant agencies to assist carers.
- Contact patients’ aged over 75 who have recently been discharged from hospital to arrange for Nurse, GP and/or Pharmacist review.

We are a friendly practice based in the semi rural town of Amesbury. We have approximately 11,185 patients of varying demographics. We have 6 GPs, 2 Salaried GPs, 1 GP Retainer, Physician Associate, 5 Practice Nurses, 3 Health Care Assistants and supporting Admin/Receptionists.
