We are seeking an enthusiastic, dynamic and motivated Band 7 Senior Practitioner. You will join an innovative, exciting and established service in Milton Keynes.

You act as a specialist clinical practitioner, provide support to colleagues and GPs with patients with complex presentations, deputise for the team manager when required and provide regular supervision of staff.

The Band 7 Senior Practitioner role will be Full Time and will involve management and supervision of B6 PCP workers and ARRS roles, deputising for the Team Manager in their absence. The postholder will provide support and consultation for patients with complex presentations, which may take the form of supporting colleagues (PCP and GP) or providing interventions.

The postholder will hold a caseload of patients in a specified clinic, undertaking psychosocial assessments, identifying appropriate interventions for new patients and supporting patients who have stepped down from secondary care who may require transitional support or review of their mental health. The postholder will work extensively alongside GPs and practice staff with support from the local mental health service.

The postholder will need to have excellent assessment skills, be able to work autonomously and be able to forge positive working relationship with other health professionals, practice staff and clients. Postholder will have excellent working knowledge of wellbeing and mental health care and the mental health needs of dynamic and changing population.

To manage and supervise B6 PCP workers and ARRS roles.

To providing support and consultation for patients with complex presentations. This may take the form of supporting colleagues (PCP and GP) or providing interventions.

To hold a caseload of patients in a specified clinic, undertaking psychosocial assessments and identifying appropriate interventions for new patients

To deputise for the Team Manager in their absence.

To forge a culture of partnership between primary care, specialist mental health services, social care and the third sector to enable a better understanding and links to all services available

To promote recovery and facilitate self-management skills for Service Users

To work with the primary care to enable them to develop skills and confidence in assessment and treatment of mental health disorders. To advise on further management where necessary. To ensure appropriate onward referral as required.

To participate in regular Clinical and Management Supervision to maintain safe practice and high standards in Service User care.

To participate in the team’s duty screening rota, assessing referrals for suitability and redirecting where necessary

To develop and provide short-term, time-limited and outcome-focused interventions and specialist programs of care to Service Users suitable for management in a primary care setting.
