This is a very exciting opportunity to make a real difference to the people of North West Wales. This new post is part of an exciting new initiative within West Integrated Health Community in BCUHB to support patients with complex needs to remain within their own community, developing support networks for patients and carers.
The successful post holder will play a key role in ensuring the delivery of evidence based interventions for adults with complex needs working alongside other Health and Social Care partners
**The Consultant Therapist role contains the elements of**:

3. education, training and development
4. research and evaluation

However, at all times the post holder will provide /demonstrate highly specialised professional advice, consultancy, clinical expertise and leadership to patients, carers and Health Board staff.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
The post holder will be clinically responsible for the day to day management of the care of all patients within the Community Rehabilitation Team, leading multi professional teams to deliver safe and effective care and liaising with specialist services.
The post holder will have overall clinical responsibility for managing patients with complex conditions in the community, working as an independent clinician but in collaboration with medical, nursing and other clinical colleagues within and outside of the MDT. Clinical responsibility for patients may be transferred from the GP or Consultant Physician to the Consultant Therapist for care in the community service.
The post holder will be an expert practitioner in the management of complex conditions, with significantly advanced clinical skills in the rehabilitation of patients with complex long-term conditions, including where mild to moderate mental health impact on long-term conditions demonstrating an advanced level of professional autonomy and accountability in the provision of such care.
Working with other Consultants and the MDT to support the development of a rehabilitation ethos, expert knowledge & practices within the staff of BCUHB.
The post holder will work in partnership with the IHCs, Local Authorities, third sector services and Academic partners to develop multidisciplinary Community Complex Conditions services in North Wales.
