**Wellbeing Coordinator.**

Single Point of Access (SPoA) service is a partnership between Denbighshire Social Services, Betsi Cadwaladr University Health Board and the 3rd Sector. It enables information, advice and assistance about community health and social care services to be accessed by adults across Denbighshire by contacting just one telephone number.

The position of **SPoA** **Wellbeing Coordinator** is essential to promote the value of the 3rd Sector to citizens over 18 years old in Denbighshire, encouraging them to help themselves to live independently through awareness of opportunities available rather than relying on help from the statutory sector.

**will involve**:

- Working effectively within the multi-disciplinary Health & Social Care SPoA Team, maintaining relationships with other agencies and promoting the role of SPoA to third sector groups.
- Ensuring staff are aware of services and resources available within the community to enable users to self manage where appropriate.
- Promoting the use of Dewis (All Wales Citizen website) for people making decisions about themselves or supporting others with decision making.
- Ensuring the resource information available on Dewis is up to date and accurate.

**, you’ll need**:

- An ability to work efficiently and effectively either as part of a team or on own initiative.
- To be able to plan and manage your own workload.
- To be educated to GCSE level (or equivalent by experience).
- To be IT literate.
- An understanding of how to improve service quality for the benefit of users
- Willingness to work flexible hours
- Full UK manual driving licence
