We are looking for an experienced and Passionate Community Coordinator (Care Navigation Apprentice) to work as part of our Navigation services across North East England.

Care Navigation services aim to provide a seamless package of support to individuals leaving secure settings and returning to the community, reconnecting individuals with health services to reduce health inequalities and in turn reduce reoffending. The Care Navigators are embedded in integrated mental health teams and community settings, providing 1:1 and group assessment and support in a variety of settings and hold highly autonomous roles.

**How you will make a difference**:
I work seamlessly with the Care Navigators and Apprentice Health and Wellbeing Coaches, I provide leadership oversight, supervision, and support to Care Navigators and Apprentices, I support in the Case Management and Clinical Skills Supervision for the Care Navigation Service. I will support the Care Navigation and Communities Lead where appropriate. Additionally, I will ensure holistic and meaningful support to service users on their release from prison, ensuring continuity of care and fruition of release plans. I will link service users to appropriate services and support networks; and ensure that they have post release access to health and social care services in parity with the wider community; reducing the risk of social exclusion, mental health relapse or potential re-offending.
