An exciting opportunity has arisen for an experienced counsellor to join our dynamic and forward-thinking multi-professional Palliative Care Team. The team provides palliative care advice and support to adult inpatients referred to us.
- To provide specialist (level 3) emotional and psychological support to patients with palliative care needs and their families/carers.
- To provide specialist multi-professional support and education to other health care professionals.
- To contribute to service development of palliative services both internally in the Trust and also within the wider health and social care system.
- To participate in research and audit to contribute to the knowledge base for supportive and palliative care.

1. Works as an expert practitioner within the palliative care team to advise and participate in the management of psychological care for patients and families. This involves patient and/or family assessment and review in clinical areas.
2. Works collaboratively with the multi-professional team to provide specialist emotional and psychological support to adult patients with life limiting conditions.
3. Assess plan and implement effective psychological interventions where appropriate.
4. Works with professional groups and other agencies in both primary and secondary healthcare settings. Assumes a central role in problem solving and service co-ordination to ensure patients and carers receive high standards of seamless quality care.
5. To assist in the development and maintenance of a service which assesses bereavement risk factors and offers support and/or referral to other services.g
6. Manages own caseload and prioritises effectively.
7. Maintains effective up-to-date contemporaneous electronic patient records. Documents plans to guide colleagues in management of psychological issues.
8. Actively participates in multi-professional meetings.
9. Supports the development of psychological knowledge and skills within the palliative care team.
10. Participate in the palliative care team education programme as well as provide education as required.
11. Contributes to research programmes where appropriate and promotes evidence-based practice.
12. Provides formal clinical supervision to the palliative care team.
13. Participates in own supervision.
14. Develops/contributes to an information database of support services available to health care professionals and patient group.
15. Is responsible for ensuring own clinical practice complies with Data Protection/Confidentiality/Caldicott principles.
16. Actively promotes diversity, including race diversity, both in the delivery of services and in the management and development of staff within the clinical area.
17. Develops own skills, taking responsibility for continuing professional development and performance. Identifies own development needs in relation to current practice and future plans, setting personal development objectives.
18. Contributes to the development of policies and guidelines concerning the psychological support of patients and families
19. Utilises an agreed service plan as a framework for service delivery.
20. Uses data from audit/quality monitoring/patient satisfaction questionnaires/service review to inform clinical governance. Utilises information to ensure that services are responsive to patient needs.
21. Ensures that critical incidents/near misses and complaints are reported in accordance with Trust policy.
22. Establishes local and national networks, linking with other practitioners in specialist area in order to share/learn from examples of good practice
