West Dunbartonshire, United Kingdom Turning Point Scotland Full time

Job Description

**Service: West Dunbartonshire Assertive Intensive Outreach Team**

**Based: West Dunbartonshire**

**Salary: £12.00 per hour - Cost of living award pending**

**Because People Matter - Work with Us**

Do you have a passion for connecting with people to achieve their goals in life, and enjoy the sense of achievement in empowering vulnerable people?

If so, we have a fabulous Lead Practitioner opportunity for you to join our awesome team, in our **NEW West Dunbartonshire Intensive Assertive Outreach Team**. Because People Matter at Turning Point Scotland, join us to reach your full potential.

You will join a team of frontline staff to deliver efficient, high quality and person-centred care throughout **West Dunbartonshire.** You will provide support to empower people to live independently, reduce the harm of drug and alcohol use and make it a great place to work for our colleagues.

You will work primarily on an outreach basis engaging with people where they are and where they feel safe: in their own home, temporary accommodation, supported accommodation or in an appropriate community facility. You will engage with individuals through an initial contact within 24-48 hours of referral to the service and follow-up contacts until the person is engaged or re-engaged with longer term support services.

You will work with individuals who have a wide range of support needs, providing them with practical and emotional support and encouraging them to achieve their own personal outcomes in all aspects of their daily lives e.g. keeping safe, meaningful activities, community involvement, physical health, relationships, emotional health and wellbeing.

Turning Point Scotland actively encourage an excellent work life balance. As this role involves evening and weekend work, we will ensure that you are given your rota in advance to allow you to plan around your personal commitments.

**West Dunbartonshire Intensive Assertive Outreach Team**

West Dunbartonshire Intensive Assertive Outreach Team provides Assertive Outreach and Intensive Case Management to support people currently experiencing problems with their alcohol and/or other drug use.

The assertive outreach approach offers a person-centred and flexible model of service.

The service adopts a “no wrong door” approach, with multiple points of entry, including self-referral and providing an out of hours response with some overlap to facilitate follow up engagement and referrals to mainstream services and offer short periods of support to those who’s needs are not being met.

This model is designed to take services throughout West Dunbartonshire to reach people who have traditionally struggled to engage, building a trusting relationship during this period

You will be part of a team that will be skilled in delivering an assertive outreach approach that delivers a range of harm reduction interventions and overdose awareness. We need someone who shares our belief in reducing harm of alcohol and other drug use, inspiring hope and supporting people to reach their potential.
