The Assistant Practitioner will work within a busy Older Adult Community Mental Health team based in North Manchester to support with meeting the physical health needs of our client group

**Taking and recording Physiological Investigations**: The post holder will undertake physiological assessment of Older adults. They will take and record blood pressure, pulse, weight, BMI and venepuncture for required blood tests.

Monitoring of Memory Treatment medication by taking physiological observations - overseen and supported by Nurse or Doctor prescriber

**Holistic well-being**: Assessing service users holistically. Assessment lifestyles - exercise and activities, eating and drinking, smoking, alcohol and drug use and giving advice on health lifestyles and signposting our service users to other services.

Team Working The post holder will work as part of a Multidisciplinary Team and provide support to care co-ordinators and other professionals

Mental Health and Wellbeing
- The postholder will contribute to assessment of both physical and mental health needs. Focusing on the physical health and wellbeing of Older Adults with severe and enduring mental health needs

Team Working
- Participating in Duty Rota
