An exciting opportunity has arisen for a Specialist Respiratory Practitioner to join the Integrated Respiratory Team at Maidstone and Tunbridge Wells NHS Trust. This role provides an excellent opportunity for a senior nurse or AHP who has experience working within respiratory, and shape the current existing service, continuing to raise the identity and provide high quality respiratory care throughout the community and the acute trust.

The team is a forward-thinking, dynamic, developing and enthusiastic team providing evidence based Respiratory care and chronic disease patients. The Countywide Service is well established and provides an acute review of these patients, out patient clinic appointments, Community Oxygen Assessment Service and establishment of non-invasive ventilation, Complex Case Management and Pulmonary Rehabilitation across West Kent. The team actively integrates and collaborates with neighbourhood teams throughout the county and liaises with all key stakeholders to provide seamless care.

This post will be primarily in the community of West Kent as part of the Maidstone and Tunbridge Wells Respiratory Team. The post holder will be expected to support admission avoidance and complex case management. We are looking for an enthusiastic, motivated and conscientious practitioner to join the existing team on a permanent basis.

The service also offers proactive support to primary care by in reaching into GP practices to discuss, and provide joint assessments for those more complex severe COPD patients. Advice and guidance for COPD management and prescribing will be offered, and nurses will be expected to have a NMP qualification or willingness to undertake.

You will be part of our multi-disciplinary respiratory team which consists of Respiratory Nurse Specialists, respiratory physiotherapists and Assistant Practitioners. You will need to demonstrate excellent interpersonal and communication skills. You must be able to prioritize tasks and manage your time effectively and work flexibly within the team.

The team work with a range of other services, including complex care nursing, community nursing, home treatment service, rapid response, GP’s, HSCC, Voluntary organizations, social services, and acute colleagues

The Service hours are currently Monday - Friday 8am - 4pm excluding bank holidays with the aim to work towards a 7 day service.

Patients are seen in the home environment as well as clinic settings.
