You would play a key role in supporting the Personalised Healthcare Commissioning service across the region to deliver an effective, competent and consistent client-focused service.

You will assist and support in the facilitation of multi-disciplinary assessments and patient reviews in line with standard operating policy and procedures, ensuring the National Framework for Continuing Healthcare and Funded-Nursing Care is implemented and complied with, according to the principles and processes.

You should be comfortable in working autonomously but also as part of the wider team to ensure the aims on objectives of the department and wider organisation are met.

As a Band 5 Practitioner you will be using your clinical skills and knowledge to assess patients using the National Framework for Continuing Healthcare and Funded Nursing Care. You should have a good understanding and knowledge of the framework and be looking to further develop your skills and expertise in this area.

You would be managing and assisting a caseload of patients, attending multidisciplinary meetings and patient review meetings, in line with standard operating procedures and local guidance. You would need to communicate complex issues to a wide range of health and social care professionals, patients, carers and family members. Therefore, excellent communications skills, both verbal and written, are essential.

You would be supporting with the arrangement of services required to meet the individual needs of the patient. You would also be investigating eligibility against national criteria for patients who have challenged or disputed a decision. You should have experience of negotiating with a pro-active and positive approach to problem solving. The ability to analyse complex and often conflicting information from various sources, to aid decision making, is imperative, as is the ability to work under pressures to meet tight and frequently changing deadlines.
