Advanced Practitioner

**The Role**:
The Advanced Practitioners will work autonomously across Primary, Community and Acute Services during the in and out of hours period, covering seven days including public holidays. They will be responsible for the clinical decision making for the population including assessment and treatment of patients in across the Primary, Community and Acute settings.
The Advanced Practitioner will work closely with the multidisciplinary team (MDT)/medical staff including the OOH Hub and Acute Services. Communicate on a daily basis about any patient they have seen overnight/ at weekend and who needs to be followed up by the GP / Acute Services during normal working hours
The Advanced Practitioner provides assessment, clinical care and management of both stable and acutely unwell patient and his/her family, making complex, autonomous decisions using advanced skills and knowledge, having overall responsibility for the assessment of care needs and the delivery of advanced care. Medical telehealth/telephone support will be available when required from an identified source.

**What we'll need you to bring**:
- Registered Nurse / AHP / Pharmacist with valid NMC/HCPC / GPHC Registration.
- Educated to/working towards/operating at Masters Level.
- Post Graduate Diploma in Advanced clinical practice and expertise within speciality /
- area of practice that demonstrates the required breadth of knowledge required to lead safety, effectively and efficiently.
- Comprehensive knowledge of clinical guidelines and standards within the speciality.
- Advanced Specialist knowledge and experience of relevant conditions, pathology, policies and procedures associated with the speciality/area of practice ensuring that the level of expertise can be utilised to deliver leadership within speciality.
- Evidence of further developed knowledge and experience in management and/or leadership.
- Further education/evidence of Continuous Professional Development in area of speciality including study days/courses/post graduate qualification.
- The post holder will possess excellent team-working/leadership skills and have the ability to motivate and influence others and work using own initiative.
- Excellent listening, communication and interpersonal skills and effective time management skills.
- Working knowledge of basic information technology.
**It would be great if you also have**:
- Non medical prescribing Qualification
- Undertaking non medical prescribing qualification
- Undertake mandatory online training e.g. violence and aggression; blood transfusion
- Postgraduate experience transferrable to or in primary and community care
- Evidence of management, education and training experience
- Experience in a medical/surgical environment
- Competencies in medical assessment and intervention
- Knowledge of infection controlguidelines
- Problem solving skills
- IT skills
- ILS/ALS

This role will be based in Primary Care Urgent Care Team within Almada St.
