Established in 1973, we are a rapidly expanding charity that provides support to individuals with mental health issues. We provide supported housing to 65 service users within Stafford, Cannock and Penkridge areas. We are a small, highly supportive and collaborative team.

As part of a team, you would provide support to enable individuals to live independently and be valued members of their community.

We are looking to appoint an individual with experience of working within a housing or social care setting, who knows how to keep and produce records and is able to manage risk. As part of a team, you would be customer focussed work experience and be resolution focussed.

The successful post holder will deliver Intensive Housing to support individuals with enduring mental health issues, which can sometimes be complex needs or challenging behaviours.

Need to have a Full Driving Licence and your own transport.

**1. Purpose of Job**

The main purpose of the role is to work both on a one to one basis and through a tailored Tenancy Management Plan to promote independence and enable clients to gain the skills to sustain a tenancy of their own in the future and to access a wide range of opportunities including employment, skills, volunteering, education training or work experience, overcoming barriers to sustain engagement in these activities.

Adaptable to the changing needs of the client and for the service. To make decisions within the League framework and policies and procedures.

**2. Duties and Responsibilities**
- To act as a Key Worker to an agreed number of clients, as designated by the Head of Housing management.
- To support the clients to access appropriate welfare benefits including Housing Benefit.
- To work with clients towards independent living.
- To be responsible for undertaking an assessment of the client’s needs often in conjunction with clinical staff or other appropriate agencies. This assessment should be based on sound recovery-based principles and encompass the need for social inclusion.
- To work collaboratively with external agencies, such as, CPA reviews, Care Planning and Multi Agency Public Protection meetings.
- To manage client payment plans and income of the organisation.
- To accurately keep records of client contact.
- To support individuals to sustain independent living, in line with their needs and Tenancy Management Plan.
- To enable clients to identify goals.
- To use the recovery STAR model to identify support needs.
- To support clients whilst they engage in activities in the community, as appropriate.
- To encourage ongoing community volunteering, education and employment opportunities.
- Encourage, motivate and empower service users to meet their goals and aspirations.
- To identify, monitor and manage risks appropriately.
- To liaise with other professionals and agencies to ensure that information regarding the client’s progress is shared according to the Information Sharing Protocol/s.
- To perform a range of administrative tasks, such as maintaining client records, record keeping, minutes, case reviews, answering correspondence and dealing with telephone enquiries.
- To ensure confidentiality at all times and adhere consistently to policies and relevant legislation; gaining any necessary authorisation for releasing information prior to doing so.
- To take an active part in a variety of learning opportunities and keeping a record of them (including mandatory training).
- To engage actively in supervision to identify and advise your Line Manager of your training and support needs.
- To adhere to Health & Safety, Lone Working and all other organisational Policies and Procedures.

**3. Person Specification**

**Essential**
- Experience of working within the social housing sector or care sector including completion of forms to claim benefits
- Good standard of English - written and verbal
- Full driving licence
- Basic numerical skills
- Microsoft Office skills and experience (Word/Outlook)
- Good telephone skills
- Negotiation skills
- Time management skills
- Work as part of a team
- To be able to work alone
- A successful track record of working with individuals with challenging behaviour, such as with addictions
- Knowledge of the current welfare benefits system and ability to complete supporting evidence

**Desirable**
- CIH Housing Qualification (Chartered Institute of Housing) - Level 3
- Knowledge of debt management and/or housing law
