**About The Role**:
We are looking for a CPA & Discharge Coordinator to work within our **BEAUTIFUL** Hospital in Roehampton, London. The Hospital is an Adult Mental Health hospital for men and women with severe and complex mental health conditions. The hospital has 28 bedrooms split across 4 individual units that all care and support adults who are detained under the Mental Health Act 1983, as well as patients who require detoxification from alcohol or drugs. Our facilities include, Gym and enclosed sports area, Designated art room, Therapy suite, Occupational therapy kitchen, Multi-faith room & Therapy garden.

The Care Programme Approach (CPA) is the national framework for mental health services assessment, care planning, review, care co-ordination, and patient and carer involvement focused on recovery.

**In your new role, you will**:

- Ensure that the tracking system is adequately managed and that MDT’s are fully engaged with the process and external teams are communicated to and updated.
- Support the effective end-to-end management of the process, you will ensure that across the hospital CPAs and Discharge Planning meetings are organised, managed and co-ordinated effectively

**About You**:
**You will ideally have**:

- Excellent IT skills and be keen to support the continuous improvement of our systems and processes. As a member of the Quality Team, you will have the opportunity to be involved in special projects that support the overall improvement plans for the site, and be expected to participate in our audits and quality reporting.
- Have exceptional organisational skills
- The ability to liaise effectively with patients, families, senior colleagues and external professionals.
