Associate Practitioner

An exciting opportunity has arisen for a Band 4 Assistant Practitioner to join our team on Peldon Ward at Colchester Hospital. You will be required to have the relevant Qualifications and hospital setting experience to be eligible for this post.

You will need toshow a willingness to undertake extended roles.
- Ensure that safe, effective and holistic patient care is delivered
- Assist patients to fulfil the activities of daily living
- With direction of the registered professional, assess, plan, implement and evaluate individual programmes of care in partnership with patients and relatives/carers
- Accurately observe patient’s condition and initiate appropriate action as required escalate to the registered professional
- If appropriately qualified administer medication safely to patients
- Actively participate in effective discharge planning
