Daytime Healthcare is working with a great healthcare provider who are currently looking for a Occupational Therapist to join their team.

You will be managing a designated caseload and completing assessments, planning, implementing interventions and evaluating outcomes.

**The Role;**
- To establish effective communication with patients, carers, all staff and other agencies.
- To maintain accurate patient records, including care notes, written reports and discharge summaries following hospital and professional standards
- To liaise with clinical teams and external agencies.
- Evaluate outcomes aligned to care plans

**Requirements;**
- Degree in Occupational Therapy
- Registered with the Health Professional Council
- Experience in Mental Health Occupational Therapy
- Report writing
