An exciting development opportunity has arisen within the SACT Outreach service.

We are looking for an enthusiastic individual with excellent communication skills with a passion for providing support for oncology patients, to join our dedicated team.

The Systemic Anti-Cancer Therapies (SACT) Outreach team provides SACT at nurse led clinics as well as local phlebotomy clinics, closer to patient’s homes. Bloods Close to Home, as a service, has expanding and if you were to be successful, you would be working as part of the SACT Outreach team with support from the wider SACT administration team based on Oak Road Treatment Centre (ORTC). You will be provided with full support and training.

The SACT Outreach team currently operates Monday - Friday, providing 13 Treatment clinic and 33 Local Phlebotomy clinics per week, across Greater Manchester and Cheshire. The service is operational on Bank Holidays. The service is evolving and is committed to providing high quality care to patients closer to home.

We are an innovative and forward-thinking team, which is dedicated to reviewing and developing the ways that we work for the needs of our patients.

You will support with answering the phones, supporting patients and relatives with appointments and queries within the Bloods Closer to Home service. You will support with patient recruitment to the Bloods Closer to Home services and assist with the daily operational management of the service.

DUTIES AND RESPONSIBILITIES
- Answer office telephone using effective communication skills when dealing with enquiries, providing empathy and reassurance to service users, relatives and visitors who may become aggressive, or emotionally distressed, referring them to the appropriate person or department for further assistance.
- Provide support and advice to patients and relatives, around local phlebotomy and SACT services which they can access closer to home.
- Exchange information with patients and staff, both verbally and in written format
- Access patients’ demographic details on Medway and amend as necessary.
- Communicate information between departments internally and external.
- Enter patient onto the Medway appointment system
- Provide comprehensive advice to patients/relatives regarding appointments.
- Deal with queries from patients and other service users, for example GP’s, hospital staff, social services, voluntary organisations and Trust management.
- Liaise with other members of staff regarding case notes and patient visits
- Resolves issues arising in own role, such as cancellation of patient appointments
- Uses own initiative to liaise with clinical staff where necessary regarding patient queries
- Use patient information systems to assist patients and staff.
- Work in Medway system, requiring standard keyboard skills
- Use of office equipment including PCs, fax machine, and phones.
- Maintain patient confidentiality at all times
- Follow all relevant policies and legislation.
- Post holder is expected to follow all relevant protocols, and standard practice.
- Work collaboratively with colleagues to ensure policies are adhered to.
- Undertakes surveys/audits as necessary to own work.
- Manager is available as point of reference for any queries and will meet regularly.
- Use own initiative to solve problems referring to manager as necessary.
- Plan and organise own time and workload activity with prioritisation
- Record all data in an accurate and timely manner.
- The post holder will be expected undergo relevant training and to take an active role in own personal development.
- Support the SACT Outreach team with patient recruitment and service expansion with identifying suitable patients to access closer to home services

Any other duties determined and appropriate to banding as identified by manager.
